Audition Terms & Conditions

I hereby acknowledge that engaging in classes at X-Quisite House of Dance LLC involves movement that could result in injury. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in any classes at X-Quisite House of Dance LLC. I represent and warrant that I’am physically fit and have no medical condition that prevent my full participation in any classes at X-Quisite House of Dance LLC. I agree to assume full responsibility for any risks, conditions, injuries, or damages, known or unknown, which I might incur or aggravate as a result of my participating in same.

 

I hereby acknowledge and understand that I’am knowingly and willingly relinquishing any and all rights or claims for legal action against X-Quisite House of Dance LLC and/ or any of its principals, their successors, and or assigns should I sustain any injuries of any kind while participating in any classes at X-Quisite House of Dance LLC. I forever release, waive, discharge, and covenant not to sue X-Quisite House of Dance LLC for any injury or condition which may arise by reason of my participation in any classes at X-Quisite House of Dance LLC. This waiver is retroactive to any classes taken before the date this waiver has been signed to me.

 

I have read the above release and waiver of liability and fully understand its content. I voluntarily agree to the terms and conditions stated above. I understand and accept that throughout any visits to X-Quisite House of Dance LLC I may be photographed and/or videotaped.

 

I hereby agree to allow my photo, videotape, recorded voice, or film, likeness to be used for any legitimate purpose associated to the promotional exploitation and/or publicizing of any class or event at X-Quisite House of Dance LLC, its principals, sponsors, or assignees. I hereby grant X-Quisite House of Dance the irrevocable right and permission to use photographs and video recordings of me with or without compensation to me.
